Operation Good Dog LLC Training Service Agreement

This is an agreement between Client (defined by the name on the purchase form), their dog(s) (herein after known as Dog) and Luke Braith acting as agent for Operation Good Dog LLC (herein after known as “Trainer”) pertains to the following:

TRAINING FEES & CANCELLATION POLICY: Client agrees to pay Trainer a nonrefundable fee based on the service(s) they selected. Client understands that appointments are between 60-90 minutes and may fluctuate depending on different circumstances. Trainer may be 15 minutes late depending on traffic.

RESCHEDULE POLICY: Client is allowed 3 reschedules. If Client fails to give at least 24 hours reschedule notice, or is not present at time of scheduled appointment they forfeit that lesson. It will not be rescheduled. If Client gives at least 24 hour notice then they can reschedule. Trainer reserves the right to reschedule at any moment due to weather, sickness, emergency or other personal reasons. Appointments expire 6 months after purchase unless otherwise noted by Trainer.

DOG HEALTH AND VACCINATIONS: Client warrants that Dog is physically fit and in good health, and free of fleas, parasites and infectious diseases. Client must provide proof of vaccines prior to participating in group classes or private classes. Dog must be current on all appropriate vaccinations, including bordetella, distemper, parvovirus, and rabies.

TRAINING SERVICES: Trainer agrees to provide private lessons for Client and Dog on a lesson-by-lesson basis, the goal being to teach Client how to train and work with Dog. Trainer will make every reasonable effort to help Client achieve training and behavior modification goals but makes no guarantee of Dog’s performance or behavior as a result of providing professional animal behavior consultation. Client understands that he/she and members of the household must follow Trainer’s instructions without modification, work with dog daily as recommended, and constantly reinforce training being given to Dog. Client understands that Trainer does not guarantee the results of its canine training classes.

RELEASE OF LIABILITY AND INDEMNIFICATION: Client understands that dog training may involve risks to themselves, members of family, or Dog. Client assumes all risks associated with participating in this training class and will not hold Trainer responsible in the event of injury to Client, family members, or Dog. Client agrees to assume all responsibility for any damage done to property, persons, or other dogs done by Client or Dog's actions. Client agrees and covenant for themselves, heirs, executors, administrators, and anyone else who may claim on Clients behalf to waive, release and discharge Trainer from any and all claims arising out of or in connection with or in any way related to this training class.

PHOTO RELEASE: Client grants to Trainer, and/or its representatives and employees, the right to take photographs/video of Client and/or Dog, and to copyright, use and publish the same in print and/or electronically. Client agrees that Trainer may use such photographs/video of Client and/or Dog with or without name and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and Web content.
